Felicia Jane LLC Policies

Welcome!

Felicia Jane Pilates is a small boutique studio in downtown Gillette, Wyoming. I look forward to helping each client reach their personal goals. Class sizes are intentionally small and personalized for the clients; all classes and private/duet/trio/quartet sessions are in high demand. In order to best serve my clients, I have established the following policies.

General

• Every client is required to complete a client profile before attending class. This helps me get to know the client better, as well as understand their goals with Pilates. 

• For equipment classes: If the client(s) is truly a newbie to the movement system (or to movement in general), I'll require at least one private so I can introduce the client to the equipment. 

• There are no walk-in appointments available. I am currently set up to take clients by appointment only. 

• Sessions are 45-55 minutes long. Mat Pilates sessions are 35-45 minutes long.

• Clients may schedule a private group class (duet, trio, or quartet) for equipment or mat at any time.

• Equipment Pilates classes will only have four (4) client spaces; mat Pilates classes will only have six (6) client spaces.

• For your safety, please do not enter the exercise space of the studio and use equipment without an instructor, or the instructor's permission.

• Everyone is expected to clean the equipment, mats or props that they use. 

• Felicia Jane Pilates is not responsible for lost or missing items. 

Payment

• Payment is due before or the same day of a session or class. FJP accepts cash, checks made out to Felicia Jane LLC, PayPal (@fjpilates), and all major credit cards processed via Square (Mastercard and Visa, American Express and Discover). Prices are subject to change without notice.

• There will be a $35 fee for all returned checks. 

• There are no refunds or transfers of packages. All sales are final. Classes and sessions are non-refundable. All packages have an expiration date.

• Prices are available with the schedule of classes and in the studio.

• All packages and classes purchased expire within 120 days of purchase. You can extend the package expiration date should it be needed as long as you notify me before it expires; if you notify me after it expires, you forfeit the fee. There are no refunds once you purchase a class. 

• If you purchase a package and need to put a hold on it, please contact FJP to discuss.

Cancellation and No Show Policy

• For studio organized equipment and mat Pilates classes, there is a 4-hour cancellation policy. 

• For private/duet/trio/quartet, there is a 24-hour cancellation policy. 

• If an appointment is not cancelled within the cancellation policy time, the client will forfeit the fee (be charged) for the session or class. 

• Clients can cancel by calling or texting 307.363.0124, or use the Acuity Scheduling app. 

• If part of a duet/trio/quartet/quintet cannot attend a lesson, the partner has the choice to (a) cancel the lesson or (b) to cover the costs of the session with one less person. For example, if part of a duet (2 people) has to cancel, and the other part wants to attend, they would pay for a private session.

• If a client is late to an appointment, their end time will be as scheduled without an extension. 

• If a client does not cancel, or is a no show to the scheduled class, they are subject to the full charge of the session. Clients who have purchased a package, the fee is taken from the purchase. If you do not have a credit card on your account and/or if it is declined, your account will show a negative balance and you will be required to balance this out upon your next visit. Without payment, the client cannot take another class.

• If you are ill and unable to attend your scheduled class, please contact FJP immediately to avoid the cancellation fee being charged. You are allowed one (1) illness-related cancellation per month that do not meet the cancellation policy above.

• There will be no waived cancellation fees. Due to limited class sizes, clients are asked to book classes accordingly and cancel prior to the late-cancellation window.

Liability Waiver and Informed Consent Release Form

All clients must sign a Liability Waiver and Informed Consent Release form, which is printed out below for your information.

I voluntarily consent to participate in an exercise program and/or assessment with Felicia Jane LLC, DBA Felicia Jane Pilates, also known as FJP in this document. 

I recognize that these activities may at times be strenuous. I have been advised and understand that participating in any exercise program presents some unavoidable risk of injury, especially to people who have pre-existing injuries, illness or medical disabilities. I hereby affirm that I am in good physical condition and do not suffer from any disability that would/could prevent or limit my participation in this exercise program.

I understand that a medical evaluation is advisable before commencing any exercise or conditioning program. By my participation in any of these activities, I acknowledge that I have either had a physical examination and have been given my physician's permission to participate, or that I have decided to participate without the approval of my physician. I have and will continue to keep FJP fully informed of any physical condition or disability that would prevent or limit my participation in an exercise program. 

I consent to the use of tactile cues to direct the proper body positioning of an exercise. 

I consent to photos or videos to be taken for client progress record keeping and marketing purposes.

I understand that in rare cases physical exercise can cause dizziness, chest discomfort, nausea, and joint or muscle soreness. I assume all risks involved and hereby waive, release and forever discharge FJP, action for any and all claims, suits, losses, or causes of damages, injury, disability or death, including claims for negligence, arising out of or related to my participation in any FJP exercise program or assessment. I fully understand that I may injure myself as a result of my participation in this exercise program, and I hereby release FJP from any liability now or in the future, including but not limited to heart attacks; muscle strains, pulls, or tears; broken bones; knee, lower back or foot injuries; and any other illness, soreness or injury, however caused, occurring during or after my participation in the exercise program.

In consideration of my participation in this exercise program, I, for myself, my heirs, next of kin, guardians, executors, administrators and assigns, hereby release FJP, its owners, officers, directors, shareholders, employees, lessor, landlord, trainees, and contractors from any claims, demands and causes of action arising from my participation in the exercise program.

FJP shall not be responsible or liable for any articles lost, stolen or damaged, in or about the studio or on location. Five Star LLC, who owns the Village Square Mall where FJP is located, shall not be responsible or liable for any articles lost, stolen or damaged, in or about the studio or on location.

I have read and understand this informed consent and release of liability and it accurately sets forth my intentions and I agree to be bound by its provisions.
